Responsibilities:
- Review and analyze HHC medical records for accurate ICD-10-CM and HCC code assignment
- Ensure documentation supports risk adjustment coding in compliance with CMS and payer guidelines
- Collaborate with clinicians to clarify diagnoses and improve documentation accuracy
- Maintain productivity and quality standards while meeting strict compliance and audit requirements
- Stay updated with current coding regulations and best practices
- Certified Professional Coder (CPC), CRC, CCS, or equivalent
- At least 1 year of experience in Risk Adjustment and/or HHC coding
- Familiarity with CMS-HCC models and HHC documentation standards (e.g., OASIS)
- Strong analytical skills and attention to detail
- Experience with EMR and coding software
