Visaya KPO is looking for a Care Coordinator Nurse with strong clinical experience to join our healthcare team! If you have a heart for patient care and experience in hospital or case management, we want you on board.
What You’ll Do
Manage and coordinate chronic care plans
Monitor and assess patient conditions
Work with providers for seamless care transitions
Prepare reports and patient documentation
Educate and support patients & families
Qualifications
BS Nursing graduate with active PHRN license
2–3 years hospital/clinical experience
Case Management Or Transition Of Care Experience Preferred
BPO experience, a plus
Willing to work onsite in Bacolod (Night Shift)
