Your role

As a Nurse Case Manager , You Will

• Coordinate Patient Care from SOC to Discharge with Case Manager Assistant’s help.
• Check SOC/ROC/Recert/Eval report daily and send the report to Staffing Coordinator to plot visits as needed.
• Complete Auth request in Kinnser for Auth Coordinator to follow up with insurance companies based on the SOC/ROC/Recert/Eval Report or the Continue Order.
• Check SN/Therapy End of Frequency daily to check whether the patient needs additional services or is ready to discharge with Case Manager Assistant’s help.
• Coordinate ordering of wound care supplies as indicated in the report or orders and enter desired amounts and send orders to the supply coordinator.
• Follow up on labs scheduled in the future to make sure they are done timely. Obtain lab results and fax to MD office. You may coordinate with the Case Manager Assistants for support with these tasks.
• Write physician orders, if any, for medication changes or new labs.
• Monitor Forcura documents to back up Case Manager Assistant as needed.
• Coordinate ordering of DMEs by writing MD orders for DME, then call MD office to get orders signed, then submit orders to DME suppliers, then follow up to ensure DME was delivered to patient’s home. Coordinate with Case Manager Assistants for support.
• Review or complete the Physician Orders in Kinnser/WellSky and get the co-signature from the office RN.
• Back up other Case Managers or Case Manager Assistants as needed.
• Respond to patient concerns such as frequency or timeliness of clinician visit, delivery of supplies, continuing authorization for care provided by patient’s insurance plan.
• Other duties are assigned based on the needs of the department.
